To help you feel more comfortable with your first visit we have provided a little information about us below, as well as a short checklist to prepare for your first appointment. First appointment checklist:
Please arrive 15 minutes early for your appointment.
Print and fill out the following three forms:
Bring your dental insurance card (if applicable).
Have your social security number ready as well as the number of the person who’s insurance you are under.
About your doctors
Dr. Alex McIlvaine was born and raised in Wadsworth. He graduated from the University of Kentucky in 2012. When not helping individuals maintain a healthy, beautiful smile, Dr. McIlvaine has been busy coaching CYO basketball, helping plan Wadsworth’s Bicentennial, and giving back to the community which has provided so much for him.
